Health Care Integrator - Watertown Office

Location: Watertown New York

Description: The House of the Good Shepherd is currently seeking to employ Health Care Integrator - Watertown Office right now, this position will be placed in New York. Detailed specification about this position opportunity please give attention to these descriptions. Health Care Integrators (HCI) are responsible for Health Care Integration, the B2H Waiver service through which the Indiv! idualized Health Plan (IHP) is created and managed.

Their role is to assess the need for services, develop and update the IHP, link the child and family/caregiver to the indentified services, consult with the child and caregiver on the appropriate provision of services, stabilize the environment for the child and caregiver, and advocate for the child's needs.

Qualifications:

  • A Master's Degree in Social Work, Psychology, or other realted field, or to be a licensed as a qualified Health Care Practitioner, or Registered Nurse, or a Special Education teacher; and a minimum of one year of experience providing service coordination and information, linkages, and referrals for community-based services to children with special needs, individuals with disabilities or seniors
OR

  • A Bachelor's Degree in Social Work Psychology, or other related field; and four years of experience providing service coordination.
! Responsibilities:
  • The HCI is responsible ! for monitoring the Waiver Service Providers (WSPs) work in relation to the requirements of the stated goals in the IHP.
  • The HCI is expected to intercede on behalf of the child or care giver to gain access to and/or facilitate needed services and support.
  • The HCI provides the link between the child and the WSPs.
  • The HCI must also monitor and observe the child, family/caregiver and WSPs to confirm that needed health care services and supports are delivered.
