Description: Residential Home Health LLC is presently looking of Registered Nurse / Case Manager (RN) right now, this vacancy will be placed in New York. Case Manage and provide direct care to an assigned caseload of patients to develop a plan of care with assessment, instruction supervision and provision ! of nursing treatments. Conduct OASIS assessments at appropriate time points.
Develop and implement a comprehensive, goal focused, care plan for each patient and makes updates in accordance with written orders from the physician.
Provide clinical excellence with customer service mindset to establish realistic and effective goals for optimal outcomes.
Prepare and maintain clinical documentation according to Agency policies and acceptable nursing standards.
Requirements:
Must possess a minimum of an Associate Degree from an accredited School of Nursing
Minimum of two (2) years of experience working as a Registered Nurse including some medical/surgical experience and/or an acute care or home care setting is preferred
Must hold, in good standing, a license as a Registered Nurse, issued by the State of Michigan
CPR certified.
