These positions are located in Greenville, SC and Suffern, NY.
Local travel required at least 50 - 75% of the time. Must have proficient clinical communication skills.
POSITION SUMMARY
The Embedded Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. The applicant must have the ability to integrate programs between hospital/physician offices and Aetna/ActiveHealth Management. Will assist with defining patient resources within the managed care program. Requires an RN with unrestricted active license.
