The Nurse Case Manager - MLTCP at Senior Health Partners (SHP) insures high quality community-based patient care and appropriate allocation of member services and resources. Monitors and evaluates effectiveness of the care plan to insure participant/care giver satisfaction with services. Acts as a resource to staff in the planning and delivery of outstanding patient care.

SHP is a paperless work environment requiring daily hands-on administration of multiple proprietary and packaged electronic databases including security requirement tools such as encryption. These systems include, but are not limited to, Electronic Medical Records (EMR), Sunguard Macess Service Module for electronic archiving, e-fax, VoIP, Virtual Work Platforms (using VPN), and scanning.

Essential Duties and Responsibilities:

  • Case Management of participant care; communicates with enrollment to insure a smooth transition at the start of care. Maintains ongoing telephonic communication with participants and care givers to facilitate patient care across continuums of care episodes. Educates participants and families in health management and community resources. Manages resources (transportation, home care, vendor services, DME, nutrition, etc.). Maintains oversight and communication with participants and providers during inpatient acute, sub-acute and/or long-term admissions
  • Quality Improvement; conducts performance improvements activities as needed.
Education Requirements:
Associate's degree

Other Requirements (License, Certificate, Other):
License required: New York State Licensed (RN)

Must be a graduate of an accredited nursing program

Work Experience:
1-3 years

Technical Skills:
Requires very accurate typing skills with intermediate Microsoft Word and Outlook. Must be able to frequently communicate complex information and interact through electronic documentation and tools such as multiple electronic PHI databases and security requirement tools such as encryption. Ability to work with Electornic Medical Records essential.

Physical Demands:
Position constantly sits and keys; frequently bends and walks; and occasionally reaches and travels. Must be able to occasionally lift and carry up to 10lbs. Repetitive use of both hands and feet is required.

Work Environment:
Position constantly works in an indoor environment.

Additional Information:
  • Position requires 1-3 years of MLTCP or home care experience. This must include knowledge of the concepts and philosophy of case management, utilization review and relevant standards of patient care.
  • Excellent oral/written communication and interpersonal skills essential. Ability to write notes in error-free, grammatically correct English essential. Must be able to effectively interact with members, community care providers, vendors, enrollment and member services. Candidates must possess excellent judgment in order to resolve any grievances and/or concerns about patient care and safety.
  • Experience managing large case loads and working in a fast-paced environment.
  • Position trains at our office in Canarsie, Brooklyn for approximately 8 weeks and then becomes home-based telephonic.
  • Understanding of and sensitivity to cultural differences and needs of the community are essential. Must be adept at operating within a diverse and multi-cultural work environment and community of Members such as Spanish, Chinese, Russian, Creole and Korean.
  • Telecommuter requirements: • Direct Ethernet FIOS or Cable internet connection (wireless connection not permitted). • Locked file cabinet for secure storage of equipment and paperwork such as job aides (Employee’s expense). This is a paperless environment so there is no printing when working virtually and for the security of patient health information (PHI). • Separate designated home work area away from the main flow of home traffic. • No dependent care. • SHP/HF has the right to conduct home site inspection with 24 hours of notice. • Random work activity audit reports will be conducted for all telecommuting employees analyzing performance and system activity on a given day. • The employee will be supplied with laptop, head-set for VOIP phone system, blackberry and all necessary applications for business purposes only.
Conditions of Employment:
Employee must pass pre-employment screening which includes a background check and drug testing. Employee must present documentation to verify minimum of high school educational requirement.
