This Part Time 16 hours a week grant funded position will allow a Peer Care Manager to provide education and support to older adult clients around wellness self-management. The Peer Care Manager will be supervised by a Nurse Care manager at this program site.
A Peer Care Manager will perform the following tasks: provide education and support regarding self- management of medical conditions, co facilitate groups on wellness self-management, accompany clients on medical appointments, outreach to comm! unity agencies and other tasks as assigned.
QUALIFICA! TIONS
High School graduate or GED equivalent. BA in social Service preferred. At least 1-3 year experience in social service field. Basic computer skills.
