NurseCore, an industry leader in Medical Staffing and Home Health Care, is seeking Compassionate, Reliable and experience! d RNs to provide Home Care for patients in the Capital District, New York area. At this time we are offering 40 hours/week M-F for visits, assessments, and documentation.

Perform routine nursing care as needed and ordered by physicians.
Conduct and document supervisory visits.
Prepare clinical, progress and summary reports.
Attend monthly case conferences with Clinical Directors.
Respect client's rights and confidentiality.
Requirements
Current RN State License
Minimum of one (1) year experience in a supervised setting within the last three (3) years
Minimum of one (1) year Home Care experience
Background in Medicare and Oasis preferred
