Description
Summary
The Community Residence Nurse is responsible for identifying the medical and health care needs of cons! umers and monitoring compliance with service and treatment plans. Responsibilities include developing nursing care treatment plans, monitoring the administration of medications and documenting adherence and follow-up to ensure appropriate medical care for consumers. The incumbent provides training to direct care staff (day and residential services) regarding the administration of medications and facilitating the delivery of required medical health care services. The Community Residence Nurse may provide direct assistance and support to consumers to secure required medical care and services. This position reports directly to the Director of Residential Services.
Requirements
1. Develops nursing care plans for consumers and maintains current documentation for prescribed medications and health care services.
2. Provides effective guidance, counseling, training and communications (both oral and written) to consumers’ direct care staff to ensure comp! liance with nursing care plans and the administration of medic! ations.
3. Reviews and updates the health care status of consumers and implement required changes. Initiates and documents monthly reviews of consumers’ medication administration records.
4. Establishes effective and collaborative working relationships with clinical and direct service providers both inside and outside of Arc.
5. Maintains current information related to NYS and Federal regulations and guidelines related to the medical and health care requirements for people with developmental disabilities.
6. Conducts at least weekly site visits to consumer residences to confirm accurate and safe administration of medications. Confirm security and safety requirements for the storage of medications.
7. Ability to drive to assigned home locations in Onondaga County and accompanies consumers on health care appointments and testing as needed.
8. Owns or leases a reliable automobile to provide transportation required for thi! s position. The automobile must be available to provide safe transportation for self, consumers and/or staff.
9. Must maintain job-required certifications and/or licenses.
10. Must carry an Agency provided beeper at all times during normal work hours and provide after hour coverage on a rotational basis.
11. Participates in and /or chair committees as assigned by Director of Community Residential Services.
12. Must be flexible to assess medical situations and/or injuries on off-hours at program sites.
13. Available to attend consumer’s 6 month and annual reviews.
14. Supports the Goals and Mission of Arc of Onondaga County. Ensures that your communications and personal performance reflect the Agency’s commitment to teamwork, compliance with policies and procedures and respect for consumers and staff.
15. Computer and office technology expertise that is required to fulfill job responsibilities and assignments! .
16. Attends and participates in required Arc meetings, staff! development and training programs.
17. Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies.
18. Provides other job-related support and services as required or assigned by management.
