The Wound Care Nurse Coordinator oversees and assumes responsibility for the skin integrity and wound management program. This individual will specialize in providing ! preventative skin care and wound treatment for residents and patients.

Performance Outcomes

Remains current in evidenced based practice and utilizes innovative approaches in the delivery of wound care management, necessary treatment and services to promote healing, prevent infections and prevent new s! kin impairments from developing.

Remains ! current and in compliance with Centers for Medicare and Medicaid Services (CMS) Regulations and guidelines. Remains current in best practices utilizing resources such as National Pressure Ulcer Advisory Panel (NPUAP), Agency for Healthcare Research and Quality (AHRQ), American Medical Directors Association (AMDA) and The Wound, Ostomy and Continence Nurses Society (WOCN), etc.

Develops and revises skin and wound care policies, procedures and protocols.

·

Conducts weekly skin and wound rounds with medical providers, nursing and other members of the interdisciplinary team within St. Ann’s Home and St. Ann’s Care Center.

·

During wound care rounds, performs comprehensive assessments for residents/patients with actual impairments in skin integrity to include, but not limited to pressure ulcers, venous, arterial and diabetic ulcers. Rashes, lesions, surgical wounds and any other skin impairments are followed at the d! iscretion of medical and the wound care nurse.

·

Works in collaboration with the interdisciplinary team and implements appropriate preventative interventions for residents/patients who are at high risk for skin impairment.

·

Results of the assessments are documented and integrated into a prevention plan of care, as well as a treatment plan of care for those residents/patients with a current open area(s).

·

Evaluates effectiveness of current interventions and revises the plan of care as necessary for optimal resident/patient outcomes.

·

Generates and distributes weekly wound reports to the CNE, nursing and the interdisciplinary team.

·

In collaboration with the Director of Quality identifies and analyzes trends.

·

Develops and assists to execute improvement plans and evaluate effectiveness.

·

Develops and conducts organized and comprehensive ed! ucation programs.

·

Provides education to nursing st! aff during orientation, annually and ongoing, based upon identified learning needs and opportunities.

·

Provides education to interdisciplinary team, residents/patients and their families.

·

Serves as an expert clinical resource to medical, nursing and the interdisciplinary team.

·

Works in collaboration with MDS coordinators to ensure completion and accuracy of skin and wound assessments.

·

Performs auditing of nurse skin and wound documentation.

·

Maintains Skin Integrity and Wound Management manuals. Provides education to nursing staff and interdisciplinary team based upon updates and revisions.

·

Work closely with outside vendors to ensure continued best practices in regards to skin/wound care products utilized. Initiate trials as indicated. Provides education to nursing staff and interdisciplinary team on new wound care products and treatment modalities.

Coordinates implementation of pressure relieving devices based upon clinical assessment.

·

Understanding of Medicare and HMO guidelines for wound care coverage. Manages ordering and billing for special requests to include rentals and purchases.

·

Responsible for tracking and reporting wound care costs for trending and budgeting purposes.

·

Work in collaboration with Nurse Leaders to achieve highest possible outcomes in regards to wound prevention and treatment.
