The Registered Professional Nurse is responsible for the management of patient care, utilizing the nursing process to assess, plan, implement and evaluate patient outcomes. The nurse directs and guid! es the delivery of nursing care, patient education, and all care related activities, while maintaining standards of professional nursing collaborating with other disciplines as appropriate.

Responsibilities:
1. Practices within the boundaries of the NYS Practice Act for Registered Professional Nurses. 2. Provides care according to CVPH Standards of Care and Practice 3. Provides nursing care to a patient by collecting data and performing a functional health pattern/physical assessment when patient is admitted, when a significant change in condition occurs, upon transfer to/receiving from another unit, and/or every eight-hour shift; provides care to patient requiring discharges. 4. Verifies and/or transcribes and implements physician orders. 5. Develops/Revises Plan of Care for patient to include measurable outcome goals for various diagnosis, and discharge plans; includes patient and/or family. 6. Provides basic bedside care to include, but not limited to! , bed baths, mouth care and ADLs. 7. Provides nursing care to ! patient requiring monitoring/intervention with â€Å"Point of Care†testing, to include, but not limited to, fingerstick blood samples for glucose and stool specimens for occult blood as defined by CVPH. 8. Provides care to a patient requiring input/output tubes/devices such as but not limited to NG tubes, urinary drains, surgical drains, and enteral feding tubes. 9. Provides nursing care at the bedside utilizing such equipment as, but not limited to, a Smart Pump, PCEA pump, PCA pump, Auto-Transfusion device, Sequential Compression device, Feeding Tube pump, splints, traction, or a CPM device; Follows manufacturer’s recommendations and CVPH guidelines. 10. Provides nursing care to the patient requiring medication administration (including controlled substance) by any route following defined process for MAK); Utilizes the Pyxis system as required. 11. Provides nursing care for the patient requiring blood/blood product therapy to include collection and reinfusion! of autologous blood. 12. Provides venipuncture and nursing care to the patient requiring IV therapy to include various routes (e.g., peripheral, central), methods (e.g., continuous, intermittent, IVPB, IVP), and solutions/medications (e.g., TPN, PPN, lipid infusions, medicated drips). 13. Provides care using infection control practices as outlined in Medical Center policies/procedures (e.g. Universal Standard Precautions and Isolation Techniques). 14. Provides nursing care to the patient and assistance to physician, when invasive procedures are performed at the bed side; procedures include but are not limited to lumbar puncture, chest tube insertion, thoracentesis, liver biopsy, bone marrow biopsy. 15. Provides nursing care to patient receiving sedation analgesia. 16. Utilizes resource consultant services and resource teams as approporiate (such as Foley Team, Rapid Response, chest tubes). 17. Provides nursing care for the patient with a tracheostomy, patient requiring suc! tioning (nasopharyngeal, oropharyngeal, or tracheal), or patient with a! chest tube. 18. Provides nursing care for the patient requiring remote telemetry and/or continuous SaO2 monitoring. 19. Provides nursing care to the patient requiring restraints/ protective/ supportive devices. 20. Provides care to identified safety risk patient (e.g. wandering/elopement, seizure and suicide precautions, and fall or bed entrapment). 21. Demonstrates respect for the diversity of patient population providing comprehensive holistic care for the dying patient, including post mortem care and support of the patient’s spirituality. 22. Provides nursing care for the patient requiring preventive skin care, wound management, pressure ulcer prevention/treatment and stoma care 23. Provides nursing care to the patient requiring various respiratory therapy modalities such as oxygen therapy and use of the inspirometer. 24. Provides nursing care to the patient requiring anticoagulation therapy. 25. Provides nursing care to the patient receiving chemotherapy, extern! al radiation therapy and internal radiation therapy (brachytherapy). 26. Provides nursing care of the urological patient. 27. Provides nursing care to the patient with End Stage Renal Disease, to include continuous ambulatory peritoneal dialysis, or pre/post hemodialysis care. 28. Provides nursing care to the patient with a head injury, craniotomy, spiral surgery or CVA. 29. Provides nursing care to the orthopedic patient to include, but not limited to total knee, total Hip, fracture of extremities, cast care, skeletal traction or skin traction. 30. Provides nursing care for the patient requiring pain management (surgical, chronic and terminal) according to organization and unit standards. 31. Provides nursing care for the patient requiring specimens collection. 32. Provides nursing care for the patient requiring transfer to the operating room or â€Å"procedural unitâ€, transfer/admitted after an invasive procedure; and transfer to another nursing unit. 33. Provides pr! ioritized patient/family education. 34. Conducts hourly rounds as a tea! m effort for assigned patients. 35. Performs in crisis/emergency (e.g. code 99, 66, Rapid Response, FAST Alert) while maintaining a positive/ calm approach. 36. Provides nursing care through utilization of the electronic information system, (e.g, Order Entry for orders, retrieving of lab data, etc) 37. Advocates patient rights and demonstrates a working knowledge of Advance Directive practices, organ donation and procurement as evidenced by completion of documentation as outlined by CVPH policy/procedures. 38. Documents/communicates nursing care, physiological responses to other health care providers through verbal reports and patient record(s) as dictated by policy/guidelines; Reports to physician changes/deviations from expected norm. 39. Maintains a safe environment in which to provide quality care. 40. Provides leadership and measures effectiveness of nursing care by providing supervision, delegation, monitoring/reassessment of all patient care as required. 41. Manages ! conflict/identifies ethical dilemmas and assists patients and families in resolving. 42. Supports fluctuation in patient care needs by floating when required. 43. Other duties as assigned. ADDENDUM: REGISTERED NURSE (CHARGE) Medical/Surgical Nursing Units The Med/Surg charge nurse is responsible for the direction of nursing personnel and overall coordination of efficient, effective, and safe nursing care. The Med/Surg charge nurse directs and guides the overall delivery of nursing care, patient safety and all care related activities, while maintaining standards of professional nursing. He/she collaborates with other disciplines and units, as appropriate, and utilizes the nursing process to assess, plan, implement and evaluate patient outcomes. The charge nurse is competent in all aspects of med/surg nursing care and communicates and coordinates with the other med/surg charge nurses and clinical coordinator to orchestrate the med/surg team for optimal care delivery. 1. Asses! ses patient needs/acuity, skills/expertise of staff and other available! resources to coordinate and adjust assignments to meet unit needs; assigns patient care, breaks, on duty inservices, meetings and project time. 2. Facilitates ongoing, interactive and positive communication with other nurses, physicians, management, and staff; serves as a liaison to the patient/family, providing timely updates on patient condition. 3. Serves as a resource person and role model for the nursing care team assuring compliance with hospital and unit standards and policies while helping to identify and solve unit related issues. 4. Serves as a mentor to provide learning experiences for staff and nursing students. 5. Provides professional leadership in crisis and emergency situations, maintaining good communication with the other charge nurses and nursing team members to coordinate effective use of available resources. 6. Participates in the development of standards for clinical practice and performance improvement initiatives, assists in the follow up of inciden! t reports and safety issues. 7. Assures that all signatures and required narcotic administration documentation is completed on the NAR before staff are released at the end of the shift. 8. Other duties as assigned. ADDENDUM: RESOURCE RN (Med/Surg) The Med/Surg Resource RN is responsible for providing med/surg/telemetry nursing care to assigned patients physically located within and outside med/surg units (e.g. ECC, hallways, PACU, and other designated overflow areas) during ECC alerts, capacity protocol and pending high census. He/she is responsible for the admission, discharge, transfer of med/surg patients in any physical location and special monitoring for procedures with competency/skills (i.e., chest tube insertion, sedation analgesia, bone marrow aspiration, etc.). The Resource nurse reports to the Float Pool Director and daily assignments are governed by the PCC. Refer to Part II of the RN job description with emphasis on admissions, discharges and transfers of patie! nts. 1. Provides care in the ECC to the patient during an Alert/Capacit! y drill. 2. Implements care in the ECC to the patient being admitted to an inpatient unit. 3. Completes admission process (nursing history, orientation screens, 1st orders, vital signs, IV’s, etc) for newly admitted patient for the patient in any designated overflow area. 4. Completes required audits as directed. 5. Supports fluctuation in patient care with flexibility with assignments.

Qualifications:
1. Graduate from an accredited School of Nursing; Bachelor of Science in Nursing preferred. 2. Currently licensed as a Registered Professional Nurse in New York State. 3. Meets requirements of physical/pre-placement screening. 4. Completion of Cardiac Rhythm Course within nine (9) months. As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
