all quality and timeline standards. The Consultant must be able to lead change, promote a positive team attitude and be able to effectively respond to unexpected changes or demands. Must be able to work independently and make sound judgments within the scope of license and program guidelines, understands when to seek assistance from professionals and management

as required.

Other responsibilities include:
Ensures Clinical competence/excellence of all field ! staff

Ensures productivity of care managers by tracki! ng and achieving established goals

Motivates and inspires Care Managers to achieve productivity goals by regular and timely feedback and guidance.

Responsible for maintaining Professional adherence to Program requirements

Proactively identifies, evaluates and solves problems in a logical and systematic approach.

Responsible to ensure all elements of the programs are met by approving all assessments, reviewing clinical notes and managing ticklers.

Ensures superior customer service

Adheres to all policies, procedures and standards set forth by the company, state and federal regulations

Anticipates, identifies and reports in a timely manner areas of potential risk; and plans their resolution or prevention.

Consults with Manager, Transitions Clinical Services and Corporate Leadership staff as appropriate.

Provides education and coaching to Care Managers re: disease management, Humana Service! s, Wellness Programs, and Resources as needed.

Performs any other activities as directed by Senior Bridge leadership.

Monitors Care Manager adherence to Transitions Program 5 Steps to Success timelines.

Approves member surveys, reviews Care Manager activity notes and manages ticklers.

Manages engagement, enrollments, non-enrollments and discharges of members in assigned branches

Educates and provides coaching to Care Managers re: Program process and goals, documentation in EMR Rosalind, disease management, Humana Chronic Care Programs, training resources and updates.

Qualifications

Role Essentials

  • 3 years experience in the areas of Nursing, Social Work, Long Term Care or Gerontology, preferably with care management experience
  • Valid license in appropriate discipline with no disciplinary action
  • Existing community and professional contacts within the geographic market
  • Knowledge of community resources
  • Proven leadership ability to influence, motivate, develop, and empower employees to achieve objectives with a team approach.
  • 1-2 years of supervisory experience
  • Proficient with Microsoft applications: Outlook, Word, Excel, and PowerPoint
Role Desirables

  • Bachelors or Master’s degree appropriate to clinical specialty
  • Proven ability to lead teams and build strong working relationships, internally and externally
  • Experience leading remote multi-location teams
  • Care Management Certification!
