Description: CDS Monarch is employing PT Licensed Practical Nurse right now, this position will be placed in New York. Detailed specification about this position opportunity kindly read the description below. OVERVIEW:
Provides nursing and medical related services, under the clinical supervision of RN, to individuals, and consultation to staff regarding health care related matters, and other I! ndividual Program Plan (IPP) components for which responsible; functioning as an Interdisciplinary Treatment Team (ITT) member, and assuring compliance with all applicable standards.

RESPONSIBILITIES:
Assures and carries out all medical treatments prescribed by physician/dentist.

Administers and monitors the security of medications to individuals per the policy and procedures.

Assures and monitor health care services in accordance with Agency and Program policy and procedure; serves as information resource to staff, parents, and residences.

Handles medical emergencies in accordance with agency policy and procedure.

Accompanies individuals to Emergency Department if needed and completes documentation of injury in individuals chart.

Follows through immediately on medication changes and newly written orders, calling MD to clarify if necessary.

Assures physician documents on medically related progress not! es as necessary.

Ensures orders on pharmacy prescript! ions from designated drug store, monitors prescription renewals, and assures medications are in supply at all times.

Observes each individual for health status and hygiene, and takes appropriate action if called for.

Monitors and submits requests for medical/nursing equipment according to agency policy and procedure; maintains medication storage area and medical supply cabinet.

Alerts all staff and RN regarding pertinent medical information via communication log or other agreed-upon system.

Performs direct care duties when needed, as assigned, provided medical/nursing related responsibilities have been fulfilled.

Ensures treatment are completed and recorded with full date and time in the individual’s chart.

Compares Doctor’s Orders to the individuals’ Medication and Treatment sheets from the residence, note any changes, and notifies the residence of any discrepancies. Secures physician review and sign off of orders! quarterly.

Fulfills all required documentation including at least med.-related progress notes, seizure chart, and menstrual chart updates Formulary, immunization record, etc.: maintains medical files and records under supervision of RN.

Acts as a liaison with other community service providers.

Maintains a professional demeanor in all community situations and acts as an appropriate role model in regard to dress/appearance and hygiene.

Maintains Medication Certification and fulfills education and training requirements.

Participates in reviews and weekly clinical meetings as assigned.

Adheres to all agency and program policies and procedures.

Performs all other necessary duties relevant to the position as requested by the RN supervisor.

Advises direct supervisor of any obstacles to achieving responsibilities.

ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS:
Valid LPN license

1 ! year experience with DD

Must be at least 18 years of age.
!
Reliable transportation (NYS Drivers’ Licensed required).

Ability to work independently and motivate others.

Ability to communicate effectively, both orally and in writing.

Ability to lift at least 50 lbs. dead weight.

Maintain all required certifications/training by State regulations and CDS Monarch policy.

All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Program Vice President in collaboration with Human Resources.

Attend mandatory Education and Training modules as scheduled, obtains, and maintains required certifications.
