Responsible for performing concurrent and retrospective quality and case reviews for discreet patient groups to assure tha! t evidence based best practices are implemented timely.
1.
Facilitates modifications to clinical documentation through concurrent interactions with physicians, nursing staff, other patient care providers, clinical informatics, IT staff and coding staff as necessary.
2.
Provides ongoing consultation and education of the patient care team, with respect to the application of continuous quality improvement. Develop educational programs to address changes in program requirements or untoward trends identified.
3.
Resolve the challenges/barriers to the smooth execution/implementation of appropriate systems to support quality initiatives through interaction with staff identified in #2.above.
4.
Assimilation and analysis, and presentation of data related to performance and or patterns and trends in performance concerns. Suggest know best practice solutions and/ or improvement strategies.
5.
Str! ives to incorporate the philosophy and mission of the hospital! and its goals of rendering quality care.
PRIMARY JOB RESPONSIBILITIES
1.
Daily concurrent review of charts.
Review of electronic as well as paper chart.
2.
Follow up with the staff regarding any addendums, updates, education needed.
3.
Current with CMS/TJC elements and data dictionary, correct version.
Willingness to review updates and educate the staff, set new information into motion.
4.
Data entry into the Quality Performer (Press Ganey) data center, knowledge of the data center and ability to obtain information needed for education, follow-up, data collection.
5.
Educate to audiences based on needs related to improvement strategies.
NYS RN.
BS preferred.
Three to five years experience in acute care clinical nursing. Masters level preferred.
Health care professional with at least 3 years experience in clinical review wi! ll be considered.
Quality improvement or a related field preferred.
Working knowledge of Centers for Medicare/Medicaid Services (CMS) and evidence based best practice guidelines.
Ideal candidate will have strong critical thinking skills, excellent written and oral communication skills, strong interpersonal and leadership skills; and a willingness to learn/develop project management and leadership skills.
