Case Manager â€" Registered Nurse â€" Case Management ( Catholic Health): The RN Case Manager coo! rdinates and facilitates a comprehensive plan of care to patients in acute care and primary care settings. Working as a member of the interdisciplinary health care team and in close collaboration with the Social Worker and Discharge Coordinator, the RN Case Manager utilizes technical skills, critical thinking, and interpersonal skills to develop and coordinate a needs based plan of care.
REPORTS TO: Site Manager Case Management

SUPERVISORY RESPONSIBILITIES:
None

SKILL AND EDUCATIONAL REQUIREMENTS (MINIMUM):
EDUCATION:

  • Registered Nurse with current New York State license
  • For candidates hired after January 1, 2000:
BSN or RN with BSW, BS Education, or Bachelors in health related field
  • Current Case Management certification in good standing by ANA-Nursing Case Management certification, or Certified Case Management preferred.
EXPERIENCE:
  • Three (3! ) years acute medical/surgical experience.
KNOWL! EDGE, SKILL AND ABILITY:
  • Excellent interpersonal communication and networking skills.
  • Excellent critical thinking, discernment, and clinical assessment skills.
  • Able to develop rapport with multiple health care disciplines and providers across the continuum of care.
  • Ability to relate well and establish partnerships with the physicians to maximize quality of care for CHS patients through the appropriate use of resources.
  • Ability to develop, implement, and evaluate care plans.
WORKING CONDITIONS:
  • Willingness to work beyond normal working hours to meet operational needs of the Department
  • Scheduled hours may include weekends, holidays and evening hours.
  • Travel to other Catholic Health (CH) facilities may be required
