Description: Good Shepherd Hospice is currently seeking to employ Hospice Primary Care Nurse/Admissions right now, this job will be placed in New York. The Hospice Primary Care Nurse/Admissions provides nursing care to hospice patient/resident and/or family within their home setting, focusing ! on symptom management and pain control. The quality of care provided will be consistent with the Standards of Practice determined for the Hospice Program. Responsible for screening and /or admitting prospective Hospice patients. In doing so, explains thoroughly the Hospice philosophy, available services and Advance Directives, obtains the appropriate consents, conducts a brief physical assessment of the major body systems and establishes the initial Plan of Care for the patient.
Must demonstrate ability in electronic medical record, paper record and interface with hospital electronic programs.
Knowledgeable regarding the Hospice Medicare benefit, as well as understanding of other insurances.
Ability to communicate in sensitive, professional and empathetic manner to family and individual facing end of life decisions.
Knowledgeable on pain/symptom management and end of life social/spiritual concerns.
On-call & weekend rotation.
Graduate of an! NLN-Accredited nursing program, Baccalaureate degree preferre! d.
Current registration in New York State as a Licensed Professional Nurse
One to two years recent general bedside nursing experience with good clinical skills.
Experience in home care, hospice and/or oncology preferred.
Willingness to travel to patients’ homes and provide own transportation in all types of weather.
