Affinity Health Plan, an independent, growing, not-for-profit managed care company designed specifically to serve the needs of low-income New Yorkers, needs a Sr. Manager- Performance Improvement and Health Promotion. In this visible position, you will develop, organize, implement and maintain efficient and effective Performance Improvement/Health Promotion Unit (PI/HP) projects, functions, procedures and work flows to meet internal and external requirements, coordinate PI/HP Unit activities with internal and external customers, and supervise the PI/HP Unit staff.

As part of your responsibilities, you will:
Develops, implements and directs procedures, work flows and timelines for all Performance Improvement/Health Promotion Unit projects/functions including, but not limited to, QARR/HEDIS chart review process, clinical studies, externally mandated projects, provider and member education, PI Team initiatives, provider quality monitoring program components and all other performance improvement activities; evaluates and ensures timely, efficient and effective completion of all PI/HP Unit projects.

Oversees the design, development and implementation of work plans for QARR/HEDIS chart review processes and all other PI/HP Unit projects and monitoring activities; collaborates in the development and implementation of protocols for clinical studies.

Provides leadership for provider partnership collaborations involving targeted PCPs and other providers throughout Affinity’s network; oversees and conducts health promotion/disease prevention projects with health care providers and appropriate internal staff.

Tracks and trends all internal and external PI/HP data; prepares and presents reports for internal and external committees and customers; prepares and maintains minutes; maintains, reviews and revises all departmental policies, procedures and protocols.

Serves as staff to the Clinical Affairs Committee; coordinates and participates in the Plan Quality Improvement Program as a leader/facilitator for QI initiatives; functions as liaison with Operations and Service Departments of the Plan for all multidisciplinary activities.

Hires, mentors, counsels, trains, guides and manages individual and team performance. Assure staff accountability through routine observation, reporting and performance measurement, and through timely, thorough preparation of performance appraisals for direct staff support.

Conducts/provides staff training and development activities to achieve objectives of orientation, knowledge and skill enhancement, proficiency/productivity and professional growth for PI/HP Unit staff members.

Assists in preparation of, and monitors compliance with, the Quality Management Department budget; prepares and submits draft of annual work plan and evaluation for the PI/HP QI Program and its component activities.

Represents the Plan with the provider community and effectively interfaces with health care providers and Affinity departments as a resource, educator, problem-solver and collaborator for all quality management activities; demonstrates tact and diplomacy in all interactions; builds consensus and negotiates mutually acceptable resolution to issues.

Represents the Plan with regulatory agencies by executing all mandated activities and projects to a timely and successful conclusion, meeting deadlines as well as standards for performance, actions and outcome and maintaining positive, mutually beneficial relationships with representatives of agencies and other customers.

Demonstrates proficiency with the principles and methodologies of process improvement. Applies these in the execution of responsibilities in support of a process focused approach.

Performs other duties as necessary or assigned.

Requirements for this position include:
Registered nurse in NY State with BA/BS degree; MA/MS strongly preferred

2 to 3 years clinical experience

2 to 3 years quality management experience, including MCO-based

Extensive knowledge of QM/CQI principles, NYSDOH and NCQA/JCAHO standards and regulations

Experience with reporting NYSDOH QARR and conducting QI studies

Advanced organizational and management skills demonstrated by the effective management of activities

Advanced interpersonal skills characterized by the use of tact and diplomacy

Strong verbal and written communication skills as demonstrated by the ability to communicate clearly and concisely

Literacy with Microsoft Office applications and data management applications

Experience with data management and reporting from clinical databases
