St. Ann’s Community is a not-for-profit organization providing a varied range of services and care levels designed to meet the individual needs of older adults. St. Ann’s offers several independent and retirement living options, short-term rehabilitation, transitional care, Adult Day Services, Assisted Living, specialized memory care and 24-hour skilled nursing care.
The Nursing Care Coordinator will report directly to the! Manager of Nursing Services.

Job Description:!
The primary purpose of this position is to supervise and coordinate resident care activities, while acting as a resource person and role model to staff members.

Clinical:

  • Oversees Team Leaders assigning of team responsibilities and coordinates implementation.
  • Supervises employees on the unit in a positive and professional manner.Communicates actual or potential employee problems to Nurse Manager in a timely manner.Takes direct action as needed.
  • Evaluates nursing activities to ensure residents care, staff relations and efficiency of service.
  • Observes nursing care and visits residents to ensure that nursing care is carried out as directed and treatment administered in accordance with Physician’s orders and ICCP.
  • Collects resident assessment information on the MDS form and provides leadership to care planning with ICCP team in formulating nursing care plan, or delegates activities to qualified RN.Monitors ! plan effectiveness.
  • Performs administrative duties such as completing necessary medical forms/reports, evaluations, studies, charting, etc., or delegates activities to qualified RN.
  • Oversees maintenance of Resident’s clinical records, reflecting current status and maintaining confidentiality of all Resident care information.
  • Assures the collection of resident condition information and communicates information to appropriate nurse staff.
  • Accompanies Physician on rounds, and keeps staff informed of orders concerning residents as assigned.
  • Continues to improve clinical processes through Quality Improvement.
  • Performs all duties of a Registered Nurse.Serves as on-unit expert and resource.
  • Meets with Residents & family members on a regular basis.
  • With Food Services & Clinical Nutritional Services, provides for nutritional aspects of resident care, including monitoring of nutritional status, feeding techniqu! e.
  • Actively participates in interdisciplinary discharge planni! ng with ICCP team.
  • Completes or delegates to professional staff, appropriate incident reports in the event of an accident or incident involving Residents, staff, volunteers or visitors on the unit.
  • Reviews Nursing documentation for accuracy and completeness re: Resident care and response to care as necessary.
  • Interprets safety regulations to staff and monitors performance in the use of medical equipment and supplies in providing Resident care.
Staffing and Personnel Responsibilities:
  • With the Manager of Nursing Services, provides for the development of staff through performance feedback and educational offerings.
Leadership:
  • Assists in staff meetings to discuss resident care and unit community development.
  • Participates in hiring, disciplining and terminating of staff.
  • Assists in employee performance evaluations and makes recommendations re: disciplinary actions.
  • Acts on behalf of the Nurse Manager in their absence.Responsibilities include, but are not limited to monitoring staffing for all (3) shifts, disciplinary issues, etc.
  • Makes written and oral reports/recommendations to Nurse Manager concerning operational improvement opportunities.
  • Reports any potential Resident abuse immediately including rough treatment, harsh or inappropriate language, etc.
  • Assures the orientation of newly assigned personnel.
  • Responsible for resident care 24 hour a day/7 days a week.
  • Shares on-call responsibilities for off-shifts, weekends and holidays with the Nurse Managers, to ensure Nursing coverage.
  • Maintains proficiency in geriatric clinical nursing skills, serves as a resource to nursing staff and collaborates with Staff Education Coordinator regarding staff development.
  • Deploys departmental objectives through regular meetings.
  • Monitors own professional goals and objectives,! reviewing quarterly with Nurse Manager.
  • Promotes continuous i! mprovement in the provision of resident care through initiative and innovation.
  • Maintains professional clinical standards, monitored through positive resident outcomes.
  • Makes independent decisions regarding nursing care.
  • Serves on various committees within St. Ann's as assigned.
  • Participates and assists in departmental studies and projects.
  • Attends and actively participates in workshops, seminars and inservices to continue professional growth, minimum 16 hours annually.
Miscellaneous:
  • Interpret facility policies and procedures to Residents, families, visitors, staff and volunteers.
  • Investigates and resolves complaints and refers unusual problems to Nurse Manager as needed.
  • Reviews complaints and grievances and makes necessary oral/written reports re: correction.
  • Performs other duties as assigned by Nursing Management Staff.
Qualifications

Education:
B.S., A.A.S. or 3 year diploma; three to five years related experience and/or training; or equivalent combination of education and experience.Some computer knowledge preferred.

Supervision:
Directly supervises & manages Team Leaders who supervise CNA’s.Assists in the overall direction, coordination, and evaluation of assigned unit.Carries out supervisory responsibilities in accordance with the organization's policies and procedures.Responsibilities include training staff; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.Supervise Nursing staff performance and other staff to ensure that care is provided according to established policies and procedures.

Specific Requirements:
Current/valid NYS RN licensure.
