Works in collaboration with Care Team Registered Nu! rse and Care Team Member Services Representative to collaborate on the plan of care for the member, and also monitor effectiveness of the care plan and promote member satisfaction with the service provision.
Responsibilities:
Responsible for care management as a team member for an assigned panel of members. Acts as a primary care manager for members with moderate or greater social needs. Documents effectively in the care management record
Experience in working in a case/care management setting, multi-cultural setting, experience in community health practices, experience with adults with chronic disorders, the frail elderly and the developmentally disabled.
Education Desired

Master's Degree in Social Work required
Experience Requirements
*
At least one (1) year of experience in a community based program caring for older adults required

Demonstrated skills in planning, organizing and directin! g the work of others required
*
Ability to work effect! ively with adults with chronic disorders, the frail elderly, and the developmentally disabled
