We want you to join our rapidly expanding firm, assisting us in offering member evaluations at our conveniently located member centers. We are looking for nu rse practi ti oners to join our member center team to complete full history and physical encounters for patients, on behalf of our clients. At the time of each encounter , the Nurse Practitioner will make general recommendations to members intended to improve members' knowledge of their chronic condition(s), such as information concerning recommended testing. The Nurse Practitioner will not be responsible for prescribing or changing a member's treatment plan. This position offers the opportunity to do these encounters on a full time or part time basis.
Duties and Responsibilities:
- The Nurse Practitioner will be responsible for the comprehensive, accurate and thorough evaluation of the assigned member population, including timely completion of all required evaluation documentation (paper or electronic).
- The Nurse Practitioner will ensure that all pertinent and active medical conditions are documented in the medica! l record in a manner compliant with CMS, Inovalon and client r! equirements.
- The Nurse Practitioner will recognize emergent or urgent situations requiring escalation and take appr opriate action as specified .
- The Nurse Practitioner will ensure that adequate communication of recommendations are made to the p rimary care provider .
Requirements:
- Active un-encumbered license to practice nursing in the state where the position is located.
- Nurse Practitioner Certification from either the ANCC or AANP.
- Compliance with recommended vaccinations and preventative testing (such as TST)
- Must be able to effectively communicate with elderly and chronically ill patients and families.
- Familiarity with ICD-9 and CPT-4 coding.
- Bi-lingual or multi-lingual a plus.
- Current BCLS
- Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial.
- Geriatric or adult medicine specialty a plus
