LPN-STAFF NURSE I-ENRICHED LIVING

Description: Unity Health System is looking of LPN-STAFF NURSE I-ENRICHED LIVING right now, this career will be placed in New York. Records and tracks medication of each resident who is staff assisted with medications; trains and monitors residents who are self medicating.
Orders medications and checks ! for outdated or discontinued drugs according to regulations. Night staff audit for expiration of medications and report to the LPN once a week.
On a quarterly basis, documents nursing summary notes and updated information on the residents in the REPS System.
Acts as liaison between the family and physician for ongoing resident medical problems.
Contacts physician regarding medical questions of residents, obtains clarification of new orders as needed.
Acts as responder with Personal Care Assistants for providing triage in emergency and non-emergency situations and completes appropriate incident reports as required.
Assists and directs resident care and ensures all supplies and appropriate equipment are available.
Assists with functional assessments prior to move-in and annually.
Rotates 24 hours On Call schedule with Program Manager for emergencies.
Coordinates first aide, CPR and other health/safety tra! ining with Personal Care Assistants as needed.
Acts a! s facility health educator.
Assists Program Manager with updating and revising medication procedures as needed.
Acts as a Personal Care Assistant during a.m. shift when needed.

Preferred Education
Graduate of an approved LPN School.
Preferred Experience
One year of acute care nursing.
One year of geriatric or enriched/adult home experience.
Required Certification/Licensure
Current license to practice as a Practical Nurse in the Sta! te of New York.
Current basic CPR certification.
First Aid Certification.
Physical Demands
May be required to lift up to 50 lbs; assisting devices or other assistance recommended.
