Exercise Physiologist (Albany, NY)
Responsible for providing guidance to disease management members who request assistance in starting an exercise program. Primary duties may include, but are not limited to:
Collaborates with nurse case managers in managing disease management health goals of members related to exercise.
Calls disease management members and assists with member inbound calls.
Implements the primary care physicians exercise recommendations providing safe and effective exercise instruction and guidance.
Facilitates understanding of nationally recognized Exercise and Wellness guidelines related to ch! ronic conditions such as heart disease, diabetes, Asthma, COPD! , hypertension, hyperlipidemia, arthritis and low back pain.
Provides telephonic health coaching, education, assessment and support of behavioral changes to promote self-management of chronic conditions.
Provides members with education materials related specifically to member score condition.

Job Qualifications
Requires a BS in exercise science, exercise physiology, kinesiology, physical therapy or physical education; 3 years of experience as an exercise specialist, exercise physiologist or fitness instructor/trainer developing exercise prescriptions for special populations; or any combination of education and experience, which would provide an equivalent background. 1 year of car! diac rehab, pulmonary rehab, and physical therapy experience in clinical setting preferred. Certification by the American College of Sports Medicine (ACSM) as a Health Fitness Instructor preferred. MS in a related field and membership in American Society of Exercise Physiologist preferred.
