ESSENTIAL DUTIES & RESPONSIBILITIES: The Coordinator of Service Coordination must be able to consistently and successfully perform all essential fun! ctions of the position.
1. Directly supervise Service Coordinators, MSC Intake staff, and Medicaid Service Coordination Supervisors.
2. Ensure staff compliance with OMRDD and other pertinent Laws and Regulations
3. Attend administrative meetings
4. Monitor and assign caseloads to Service Coordinators and MSC Supervisors
5. Attend meetings/trainings pertinent to Medicaid Service Coordination programming
6. Provide on-call 24-hour phone coverage at least one week per month
7. Maintain a caseload of high priority service coordination cases
8. Perform any other duties as requested by your supervisor
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of position skill and/or ability required.
- EDUCATION and/or EXPERIENCE - Bachelor Degree in Social Work or related human services; 2 years experience workin! g with families; at least 2 years working as a service coordin! ator; At least two years experience as a Medicaid Service Coordinator at Summit Educational Resources. Proven knowledge of OPWDD regulations, including eligibility requirements for MSC and HCBS Waiver; proven proficiency in provision of MSC services and in enrollment processes. At least two years of supervisory experience preferred. Meet the qualifications of a Qualified Mental Retardation Professional(QMRP): (1) At least one year of experience working directly with persons with mental retardation or other developmental disabilities; and (2) Is one of the following:
(ii)A registered nurse
(iii) An individual who holds at least a Bachelor’s Degree in a professional category.
- CERTIFICATES, LICENSES, REGISTRATIONS Etc. - Mandatory OMRDD Core Service Coordination training. Must be cleared through NYS OMRDD criminal background check unit prior to first day of employment.
- COMMUNICATION SKILLS - Comma! nd of the English language exhibiting the ability to verbally communicate, read and interpret general education periodicals/professional journals, technical procedures, or government regulations; write and comprehend routine instructions, short correspondence, reports, and IFSP's/ISP's, etc. Ability to effectively present information in one-on-one and small group situations to parents, students, and other employees of the organization etc.
- OTHER SKILLS & ABILITIES - Ability to define problems, collect data, establish facts, draw valid conclusions, solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret an extensive variety of instructions furnished in written, oral, diagram, or schedule form.
