Registered Nurse - Community Residence FOUR DAY WORK WEEK AVAILABLEJob Summary: The Nurse will provide oversight and coordination for the healthcare of the residents of Glove House, Inc., with primary ! attention for those residing at the Community Residence. The Nurse participates as a member of the resident's treatment team through development and oversight of a comprehensive nursing and/or healthcare plan for each resident. The Nurse is a member of the Community Residence Co-leadership Team. The Nurse ensures that all nursing standards and regulations are met. The Nurse is responsible for procurement of healthcare supplies, monthly inspections related to medications, maintenance of clinical records, and coordination of healthcare appointments. The Nurse serves as a training and educational resource for the residents, their families, and program staff in groups or with individuals. The Nurse is responsible to adhere to and to ensure that all regulations and standards regarding residents' health care are carried out as set forth by Glove House policies, and procedures, the Office of Children and Family Services, the Office of Mental Health and Medicaid. The Nurse is direc! tly responsible to the Nurse Manager. Duties & Responsibilitie! s: 1. Provides direct nursing care of the residents, meeting, directing or facilitating all healthcare needs. 2. Performs and documents an initial nursing assessment upon admission of a resident. 3. Performs and documents all nursing assessments and healthcare issues during the resident's stay. 4. Assures the facilitation of medication training and education for program staff, including curriculum that is utilized. 5. Assures, and facilitates when appropriate, health screenings for all residents upon admission to the program. 6. Assures the timely completion of an initial and periodic comprehensive health evaluation including medical, dental, mental health, developmental, and alcohol and substance abuse assignments. 7. Facilitates by preparing for and attending (as appropriate) all healthcare appointments with residents such as, but not limited to: psychiatric, medical, dental, laboratory work, psychological testing, healthcare testing, vision, and psychotherapy. 8. Evaluat! es residents provides nursing documentation following events of AWOLS, therapeutic holds, or other events, assisting in investigations as indicated. 9. Monitors the effects of medications administered to residents in conjunction with program staff and regularly interviews residents to ensure medications are taken by participants as prescribed. Ensures renewal of necessary prescriptions and disposal of out-dated or discontinued drugs according to agency policies and procedures. 10. Assesses injuries and ailments and coordinates the appropriate treatment and documentation. 11. Performs medication administration audits assuring the proper completion of record keeping as required by the program, agency and state regulatory agencies. 12. Initiates specialty referrals for conditions not manageable by the primary care provider. 13. Develops and/or implements healthcare programs as directed. 14. Performs all job duties with the Agency's mission, vision, and values in mind. 15. Part! icipates in all agency nursing meetings and keeps apprised of all high ! profile healthcare situations with residents within the agency through daily reports. 16. Drives on agency business as required by job expectations. Program Coordination 1. Participates as an active member of the Co-leadership team of the Community Residence. 2. Provides healthcare education for residents, parents, and program staff individually or in a group setting as appropriate. 3. Provides nursing care for other agency homes or residents as requested. 4. Attends multi-disciplinary inter-agency meetings or submits a healthcare summary and plan. 5. Works with contractors to determine the process of coordination between the primary care provider and specialty care providers. 6. Assures the inclusion of appropriate program and provider staff in the multi-discipline treatment planning for residents with complex chronic mental health and behavioral problems. 7. Participates in the agency's on-call-nursing service as directed. 8. Performs other duties within the organization ! as directed by manager.
Education: Must be licensed by the New York State Department of Education to practice as a Registered Nurse. Experience: A minimum of one year experience working as a nurse. Experience with the psychiatric care of adolescents preferred.Must have reliable transportation and valid NYS driver license
