POSITION SUMMARY:
The coordinator is responsible for collaboration with interdisciplinary team members while implementing the nursing role in end-of-life care. Responsible for ongoing staff education and development in caring for patients at end-of-life. Serves as a resource throughout the hospital in formulating plans of care for improved end-of â€"life care. Applies knowledge gained from palliative care research to end-of-life care and education.

EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:
Bachelor prepared Registered Nurse with a minimum of six months progressive experience in the field. Ideally, the candidate will have some supervisory experience. Licensure as required by regulatory agency. Committed to obtain National Certification in ELNEC, palliative care or hospice within two years. Participates in orientation and continuing education, updates, and maintains knowledge and skills related to specific areas of expertise.

SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS:
Strong written and oral communication skills. Strong interpersonal, motivational and conflict resolution skills. Strong participatory management, team building and administrative skills. Broad based knowledge of related regulatory compliance requirements.

WORK ENVIRONMENT AND HAZARDS:
Exposure Class I or II, service specific â€" limited exposure to situation which could cause serious illness or injury (II), routine or potential exposure to blood, body fluids, excretions or secretions (I).

PHYSICAL DEMANDS:

Works closely with service management regarding daily operational issues including short, mid and long range planning and coordination of overall program.

Operational: Ensures the daily operations of the service, including but not limited to personnel, safety, coordination, communication, project/work flow and performance of staff duties as required and assigned. Serves as a resource for information about the service in particular and provides consultation as needed. Assists in the recruitment, development, retention and evaluation of staff. Identifies barriers to palliative care program and actively works to facilitate change. · Communications: Facilitates communication between patient, family, colleagues to aid in coping with suffering, loss, and bereavement in end-of-life care. Communicates effectively and compassionately with patient, family and health care team members about end-of-life issues. Effectively educates the patient/family/staff in specialty area.

Researcher: Applies current concepts and findings from research and/or studies to practice. Participates in Performance Improvement and monitoring for palliative care service.
