The Case Manager - RN will perform the following duties:
Coordinate and procure services necessary to meet the post-acute care needs of patients identified as requiring such services
Ensure a timely and smooth transition to the most appropriate post-acute care setting
Work collaboratively with interdisciplinary patient care staff both internal and external to the organization
Participate in quality improvement and evaluation processes related to the management of patient care
Ensure maximization of provider reimbursement of patient days through timely and accurate insurance reviews
Perform other duties as necessary for the position
Requirements
Diploma or AAS Degree in Nursing will be strongly preferred for this position
The applicant must have current New York state license as a Registered Nurse
Certification from the NYS Department of Health as a Patient Review Instrument (PRI) Assessor will be highly preferred
The applicant must also have a minimum 5 years of acute care clinical experience within the past 5 years
3years of discharge planning experience and/or 3 years of utilization management experience will be highly preferred for this position
The applicant must have excellent communication, interpersonal and organizational skills
The applicant must also have knowledge of clinical standards of practice for multiple patient populations
Knowledge of post-acute care referral process for the procurement of patient services will be required for this position
The applicant must be computer proficient
Advanced skills in computer will be preferred
Knowledge of hospital reimbursement process, insurance denials and appeals process will be preferred
Emergency Department/Case Management experience will be a plus for the position
Benefit
