Position Summary:
Support patient centered care by providing personal care services and other duties to patients while admitted to WPH as directed by primary care nu! rse assigned to patient. Is responsible patient safety by providing constant observation or constant supervision of patients admitted to WPH or admitted to the Emergency Department. Assists in care activities on nursing units as directed by RN on nursing units. Provides services to multiple patients each day.

Provide personal care as well as nutritional and light housekeeping assistance to patient
Recognizes and reports changes in patient’s physical, mental and social status
Performs clinical duties as assigned such as glucoscans, TPRs, BPs, and documents results as required.- will perform duties and tasks assigned by RNs and NTs as defined in patient’s individual plan of care.
Will socially interact with patient building interpersonal relationship while caring for patient
Provides support of patient care activities on nursing units
Transports patients to test! s and procedures and will remain with patient if requested by ! RN
Participates in Performance Improvement activities as needed
Maintains appropriate documentation in EMR or other nursing unit logs
Education, Training, Licensure and Experience Requirements
High School Diploma
Successful completion of departmental orientation
