Develop an appropriate discharge plan for the patient prior to admission
Educate the patient/family on discharge planning options as wel! l as insurance benefits
Provides social work consultation, leadership and teaching functions for designated medical departments as well as for the community
Requirements
Please be advised that we are only considering individuals with the following:
LMSW or LCSW with discharge planning experience
Clinical social work knowledge and practice skills related to psychosocial aspects of illness
Knowledge of community resources
