Case Manager

Description: St. John's Riverside Hospital - Andrus Pavilion is at the momment seeking for Case Manager right now, this position will be placed in New York. Job Summary : Assists in meeting the psychosocial needs of a patient and families, through assessment of inpatients based on assignment. Patients are screened for continu! ing care needs, including emotional support, community resources needs, home care, inpatient care and health insurance issues. Educates, coordinates and collaborates with nurses, physician, continuing care coordinator and interdisciplinary members of the health care team, to assure an ongoing comprehensive discharge plan.
Qualifications: NYS licensed RN required. Three years Case Management or Discharge Planning experience required. Knowledge of medical insurance procedures regarding discharge planning and authorization required. Experience in Utilization Management, including the ability to conduct medical insurance reviews required. Ability to refer a patient for home care, including IV therapy and wound VAC care required. Strong assessment and communication skills required. Able to read, write, speak and understand the English language required. Knowledge of the services of the community, health, welfare, and social agencies required. Excellent organizational ! skills and ability to prioritize required. Effective professio! nal support with physicians, patients, family/visitors, peers and supervisors required.
Preferred Education/Experience/Skills: Bachelor's degree in Nursing or health-related field. NYS Screen Assessment Certification. NYS PRI Assessment Certification. Demonstrates flexibility and creativity. Function well under pressure. Good leadership ability and good judgment. Ability to communicate Spanish or community dominant language. Educated in the use of the particulate respirator (mask). Familiar with Medicaid and services provided by Medicaid. Knowledge of skilled nursing facility and rehabilitation placement. Hospice and/or palliative care experience.
Union: NYSNA Rate : $36.128/hour-$50.282/hour (based on years of experience)
