Review inpatient charts and act as a resource, consultant, and educator to staff in the planning and delivery of quality patient care services
Participate in the d! evelopment, organization, implementation, monitoring and evaluation of healthcare services to assure quality patient outcomes for assigned members
Manage patient care for Medicare/Medicaid members, their families or significant others and coordinate with the PCP, SCP and all members of the health care team to facilitate the patient's care across the continuum of care
Perform pre-admission, concurrent and retrospective reviews to evaluate appropriateness of admission, need for continued stay, length of stay, utilization of resources, patient outcomes, and the usage of other services post-encounter
Assure compliance, in practice, with regulatory mandates
Identify opportunities to facilitate member transfers
Document all interventions and telephone encounters with providers, members and vendors in the appropriate system in accordance with established documentation standards to ensure integrity of member services
Assess members' physical, psychological a! nd discharge planning needs through communication with appropr! iate hospital staff including attending practitioner, utilization review staff, discharge planners, social workers, and coordinate care accordingly
Requirements

Must have 2 years of clinical experience
Must have previous experience with utilization review
Previous experience in case management and guidelines required
