3p-11p with every other weekend rotation
This position is responsible for coordinating total comprehensive, patient care and psych! osocial/spiritual intervention for the patient and family in accordance with Hospice Buffalo, Inc. and HIU policies and procedures. He/she assumes accountability for assigned patients and consulting supervisor and/or director as needed. Staff RN aides in coordination of assignments for LPNs, nursing assistants and volunteers to address the patient/family needs. Care given is patient centered and goal directed to meet the specific needs of the patient/family while supporting the definition of Hospice.Performs other duties as required.
QUALIFICATIONS
Graduate of an accredited school of nursing and currently eligible for permit and/or licensure in the State of New York. Certification in Hospice and Palliative Care Nursing encouraged. A minimum of two years of nursing practice as a professional nurse is preferred; however, one year of practice as a professional nurse with additional education and/or health related experience may be considered.
